The N.C. Natural Heritage Program (NCNHP) requests locality and population information about the plants listed in the Natural Heritage List of Rare Plant Species of North Carolina, including those designated as Watch List. To provide information, please complete the Plant Survey Web Form below.

If you prefer to print out a plant survey form, you may download the form as a Word document and email the completed form to natural.heritage@ncdcr.gov or mail it to N.C. Natural Heritage Program; MSC 1651; Raleigh, NC 27699-1651.

You may also contribute data via iNaturalist. Please visit the NCNHP profile on iNaturalist and read the journal posts to learn more about how to join the project and share data with NCNHP. When submitting data via iNaturalist it is helpful to complete an associated Plant Survey Web Form (below) with additional information about the observation, including details about the size of the plant population and habitat description.
